Annual evaluation of a soldier in the Navy is made based on the information submitted via this NAVPERS 1616/26, also known as Evaluation Report and Counseling Record. Soldier’s actions and preparation over the reported year are to be evaluated by the senior rater from the special commission.

This form should be submitted on the annual basis. The exact time of the evaluation of a particular soldier is reported individually so there is no specific deadlines for filling out this form.

Name, Rate, Ship / Station, Promotion Status should be provided in order to complete this form.
